$40 - Itemized billing and records from any provider nationwide such as, emergency rooms and physicians, radiologist, chiropractors, medical practitioners, diagnostic testing facilities, etc. Our requests and authorizations are strictly reviewed for accuracy to avoid any delays, cancellations, or errors. Medical records are requested via HITECH request to eliminate any unnecessary charges. We will also request any waivers or courtesies on your behalf to expedite delivery.

$40 - Police Records such as police reports, dispatch recordings, dash cam videos, service call notes, photos, data and other public records related to your case.

$40 - Employment records such as wages, hours, lost time from work, nature of employment, “loss of wages” letters from employers, and any other documents that may be helpful to your case.

$40 - In the event that you need a notary on site, we can arrange it for you.

$50 - We will recover any and all statutory liens from anyone with right to payment pertaining to your case including, but not limited to, Medicare, Medicaid, TriCare, private insurance carriers, child support liens, etc.

$100 - To ensure that settlement proceeds are distributed fairly, we observe all statutes in your state that allow for reductions, or caps, when negotiating your liens. We will submit all of your reduction requests to anyone with right to payment including, but not limited to, Medicare, Medicaid, TriCare, private insurance carriers, Office of Attorney General, etc.
